Job Description​ 🌟 We're Hiring: Advanced Practice Provider – Palliative Care (Full-Time Days) Join our compassionate and collaborative Palliative Care Team and make a meaningful difference in the lives of patients and their families. Position Details Full-Time Day Shift Schedule: Monday–Friday, 8:00 AM – 5:00 PM Primary Location: Saint Luke's Plaza Opportunity to float to additional sites as needed About the Role As an Advanced Practice Provider (APP) , you will provide advanced clinical care to assigned patient populations while serving as a key member of an interdisciplinary team. Responsibilities include: Assessing, collecting, and analyzing patient health data Developing, implementing, and evaluating individualized plans of care Coordinating comprehensive patient care and case management services Providing expert clinical consultation Utilizing prescriptive authority, procedures, and treatments in accordance with state and federal regulations Educating patients, families, and healthcare staff Managing high-risk patient populations Supporting quality improvement and research initiatives Promoting positive patient outcomes and enhancing care delivery systems Call, Weekend & Holiday Requirements Call coverage: One week (7 consecutive days) at a time Currently, call and weekend coverage are provided only at Saint Luke's Plaza Call and weekend commitment: Approximately 1–2 times per quarter Holiday commitment: One holiday per year Why Join Our Team?
