Organization: Hematology/Oncology Job Summary: The Advanced Practice Provider (APP) plays a vital role in delivering specialized care and managing the health needs of patients in the ambulatory settings. With advanced training and expertise, the APP focuses on subspecialized care, including chronic illness and disease management, ensuring continuity of care throughout the patient’s healthcare journey, which may span multiple years. The APP is integral to supporting patients across the continuum of care collaborating with multidisciplinary teams to achieve optimal patient outcomes. This role specializes in cancer survivorship, with most patients having completed treatments for genitourinary or breast cancer. The APP provides expert consultation, treatment summaries, and recommendations for ongoing care. Additionally, the APP will work in collaboration with physicians to care for patients currently on active treatment for genitourinary or breast cancer at the Belle Meade clinic. . PREFERRED QUALIFICATIONS Oncology experience is preferred (not required) DEPARTMENT SUMMARY The Division of Hematology and Oncology at Vanderbilt University Medical Center conducts research and treats all types of adult cancers including solid tumor cancers and hematological (blood) disorders, as benign hematologic disorders such as coagulation disorders and non-cancerous blood diseases.   With over 80 faculty members representing a broad range of professional expertise, our dedicated team of professionals is committed to hematologic and oncologic care through: Research to better understand and treat cancers and benign hematologic diseases  Translating VICC laboratory research into therapies to help people with cancer  State-of-the-art patient care and personalized cancer therapy  Clinical trials, including Phase I drug development with subsequent Phase II and Phase III testing  Training future hematology and oncology doctors and researchers  Cancer chemotherapy, pharmacology, and biological therapy with an emphasis on solid tumors  Treating bleeding and coagulation disorders, anemia and other disorders of blood  REACH for Cancer Survivorship | Vanderbilt Health Nashville, TN POSITION SHIFT 4 days per week in clinic (8am-5pm) with 1 administrative day Provide shared coverage of MRI and infusion amongst APP team (typical day coverage hours 8 am-6 pm). Location: VICC Belle Meade No call, nights or weekends KEY RESPONSIBILITIES Obtains and documents a health history; performs and documents complete, system-focused, or symptom-specific physical examination, assessment and plan of care. In collaboration, provides healthcare services for primary, acute and complex care of patients and manages patients’ overall care, identifying expected outcomes for diagnoses. Orders, performs, interprets and collects data using appropriate assessment techniques, relevant supporting diagnostic information and diagnostic procedures where indicated. Forms differential diagnoses and treats acute and chronic conditions. Prescribes therapeutic interventions for both pharmacologic, non-pharmacologic and surgical, needed to achieve expected outcomes. Utilizes evidence-based, approved practice protocols in planning and implementing care. Initiates appropriate referrals and consultations Provides specialty specific consultation services upon request and within a specialty scope of practice. Uses health assessment skills to differentiate between normal, variations of normal, and abnormal findings. Employes screening and diagnostic strategies in the development of diagnoses. Prescribes medications within scope of practice. Manages health/illness status of patients and families over time and across the continuum. Provides patient/centered care recognizing the patient or designee as a full partner in decision making. Creates a climate of patient/centered care to include confidentiality privacy, comfort emotional support, mutual trust, and respect.
