About the Role We're seeking a visionary Director of Value-Based Care Pharmacy to build our pharmacy program from the ground up. This is a rare opportunity to architect the entire clinical pharmacy infrastructure for a rapidly growing senior-focused primary care organization — from formulary strategy and Part D cost optimization to HEDIS quality improvement and provider education. As our first pharmacy leader, you'll collaborate with the Chief Medical Officer and physician teams to design the strategic roadmap for medication management across our expanding network (currently 4 clinics. You'll work closely with clinical leadership to establish collaborative practice agreements, build strong provider relationships, and create the systems that will drive significant cost savings while dramatically improving patient outcomes. Operating under collaborative practice agreements, you will directly influence medication therapy, optimize clinical outcomes, and reduce total cost of care through evidence-based, protocol-driven interventions. Reporting to our Chief Medical Officer, this role balances strategic thinking with hands-on building — perfect for an entrepreneurial pharmacist who thrives in startup environments and wants to see the direct impact of their work. You'll visit each clinic monthly to provide direct patient care for complex chronic diseases, while simultaneously developing the analytics, workflows, and team structure that will scale as we grow. This role is ~30% clinical, ~40% program building, and ~30% strategic/analytics. Compensation range: $170,000-$180,000 annually with a generous performance bonus. Key Responsibilities Pharmacy Strategy & Program Build Build and lead pharmacy program infrastructure from the ground up, including formulary development, medication management protocols, and clinical workflows Drive Part D and Part B cost optimization through therapeutic alternatives, formulary compliance, and patient assistance programs Own pharmacy-related HEDIS quality measures and medication adherence improvement initiatives Represent pharmacy at insurance partner meetings and contribute to managed care strategy Clinical Care & Medication Management Provide direct patient care for complex chronic disease management under collaborative practice agreements Conduct comprehensive medication reviews evaluating safety, efficacy, cost, side effects, and barriers to adherence, particularly for high-risk and polypharmacy patients Reduce polypharmacy and improve medication adherence through targeted interventions aligned with value-based care goals Manage chronic diseases (e.g., diabetes, hypertension, anticoagulation, heart failure) through proactive medication optimization and monitoring Order, interpret, and apply laboratory results to guide medication therapy decisions and ensure safety and effectiveness Care Coordination & Provider Enablement Improve transitions of care by identifying and resolving medication discrepancies post-discharge and reducing avoidable readmissions Partner with care teams to ensure seamless coordination between primary care, specialists, and pharmacy services Develop and deliver provider education programs on medication optimization and value-based care strategies Analytics, Performance & Impact Build analytics dashboards and reporting systems to track program impact, cost savings, and ROI Use data to identify opportunities to improve adherence, reduce cost, and elevate clinical outcomes Field Leadership Visit clinic locations regularly for in-person provider engagement and patient care Develop and mentor future clinical pharmacy team members, with the opportunity to build and lead a team as the organization scales Accountabilities Financial Performance: Part D cost reduction, formulary compliance optimization, and PAP savings generation Quality Metrics: High-star HEDIS ratings on pharmacy measures and improved medication adherence rates Program Development: Timely launch of core pharmacy programs including formulary, PAP, HEDIS tracking, and provider education Provider Engagement: Strong provider satisfaction scores and high engagement with pharmacy education programs About You Education & Licensure: Doctor of Pharmacy (PharmD) degree and active Florida pharmacist license in good standing Training: PGY1 Pharmacy Residency (required); PGY2 Ambulatory Care Resi
