Be rewarded with a unique opportunity to make a difference Competitive compensation package Tuition reimbursement for full-time staff and continuing education opportunities for all employees at no cost Opportunities for advancement Comprehensive insurance plans for medical, dental, and vision benefits 401(K) with employer match Paid time off, paid holidays, family, and pet bereavement Pet insurance As a Engage Telehealth RN you’ll contribute to our success in the following ways: Ensures virtual visits are completed accurately and timely as defined in the patient’s plan of care Supervises LPNs and manages patients care plan in accordance with MD orders Communicates with the branch clinical staff, patient physician, or to other members of the interdisciplinary team, regarding any changes in the patient’s physical or emotional condition Completes daily calls to high-risk patients and evaluates patient interactions to determine if patient needs are met, recommending appropriate and timely interventions as needed Remotely educates patients, family members and other caregivers regarding the best ways to meet patient diagnosis specific needs and administer care plan Regularly evaluates patient needs by observing signs and symptoms during virtual interactions. Evaluates patient’s social determinants of health (SDOH) to identify areas of additional support. Recommends community resources or coordinates referral to LMSW as appropriate Identifies and coordinates care for patients requiring additional services by directing them to appropriate internal or external resources Completes assigned dashboards and workflow timely. Assures all documentation is completed accurately and timely Follows regulatory requirements and timelines when assessing new patients Participates in clinical record audits, utilization reviews, Peer Review or Quality Improvement programs, as appropriate Attends required training, education, seminars, or other means of learning Maintains availability at all times during scheduled hours of work, either in person or virtually Processes all data entry information into the company or vendor software timely and accurately Participates in weekends and/or holidays, as needed and assigned. Performs other duties/projects as assigned. What is Required? Associate's degree in nursing (ADN) is required. Bachelors in nursing preferred. Minimum of two (2) years of active experience as a Registered Nurse required. Experience in Telehealth, ICU, ER, Triaging experience, Medicare/Medicaid home health care benefits, policies and procedures preferred. Current Certified Registered Nurse (RN). Reliable transportation for in-office meetings or other reasonable requests to be in office for daily tasks. Must have a dependable vehicle, a valid driver’s license, and current auto insurance in accordance with the laws of the State.  Occasional travel to local office for in-office meetings. Meets all applicable agency policies and procedures related to health screening and required testing.
