Catholic Home Care is seeking a compassionate, experienced Registered Nurse to join our team as a Home Care Transition Nurse. In this highly visible role, you'll serve as a trusted clinical liaison, collaborating with physicians, patients, families, and healthcare partners to ensure seamless care transitions while expanding access to high-quality home care services. The Home Care Transition Nurse  is responsible for assessing, planning, and developing referrals. 5 x 8 hour shifts (9:30AM to 5:30PM) - Rotating Weekends and Holidays Great Healthcare benefits, and education reimbursement of $5,250 annually Additional compensation for visits over your caseload This position will travel between Skilled Nursing Facilities Job Details Our Home Care Transition Nurses : Conduct patient assessments and develop individualized transition plans Coordinate referrals and facilitate smooth transitions to home care services Build and maintain strong relationships with physicians, discharge planners, and referral sources Serve as a key ambassador for Catholic Home Care within the community Identify opportunities for program growth and service expansion Deliver exceptional customer service and support to patients, families, and healthcare partners Collaborate with interdisciplinary teams to ensure positive patient outcomes Requirements: Current Registered Nurse (RN) license in New York State Minimum of 2 years of home care experience, including patient assessments Strong relationship-building, communication, and customer service skills Experience in healthcare marketing, referral development, or business growth is a plus Passion for patient advocacy and care coordination Posted Salary Range USD $95,000.00 - USD $120,000.00 /Yr. This range serves as a good faith estimate and actual pay will encompass a number of factors, including a candidate’s qualifications, skills, competencies and experience. The salary range or rate listed does not include any bonuses/incentive, differential pay or other forms of compensation that may be applicable to this job, and it does not include the value of benefits.
