Home Care Transition Nurse-Weekends-Farmingdale

Overview The  Home Care Transition Nurse  is responsible for assessing, planning, and developing referrals. This is an every weekend position 7:00 AM-5:30 PM on site in our Farmingdale office located at 110 Bi-County Boulevard.  Job Details Our Home Care Transition Nurses : Establish and maintain professional relationships with key physician groups and referral sources. Act as Liaison to referral sources on behalf of Catholic Home Care. Identify potential market growth opportunities and program expansion. Serve as a Customer service representative and act in a professional and supportive manner at all times. Requirements: Current New York State Registered Nurse licensure  2 or more years of home care experience preferred Customer Service and Marketing skills required Posted Salary Range USD $55.00 - USD $65.00 /Hr. This range serves as a good faith estimate and actual pay will encompass a number of factors, including a candidate’s qualifications, skills, competencies and experience.
