Licensed Clinical Social Worker, Remote

Salvo Health is looking for an experienced Lincensed Clinical Social Worker to play an integral role on our care team.  This role will work directly with patients virtually, offering personalized psychosocial support and counseling to patients to reduce symptoms and improve quality of life. The LCSW will empower patients with the knowledge and tools to make lasting behavioral changes that will improve clinical outcomes and overall wellness. You’ll serve as one of our patients’ primary success partners, with responsibilities including: addressing member health questions, providing relevant educational resources, and fostering positive member accountability.  If you have a passion for patient empowerment through a holistic-based approach to medicine, come join our team! We bring multidisciplinary virtual care to patients at community specialty care practices, particularly GI and hepatology brick & mortar clinics around the US.  We assign each patient a “whole patient” care team and offer daily access to app-based care, using Remote Patient Monitoring (“RPM”) and Chronic Care Management (“CCM”) to bill. These services are covered by most patient’s insurance, both Medicare and commercial.
