Licensed Practical Nurse ( LPN) - Per Diem

Overview At Good Shepherd Hospice, our organization promotes the physical, emotional and spiritual well-being both of the person completing life's journey and their family members by providing comprehensive and compassionate end-of-life care of the highest quality.  We are looking for a compassionate LPN with the "Hospice Heart" to join our care team who are committed to providing both medical and emotional support to patients and families. Per Diem requirement is 6 shifts per month, covering evenings and weekends. Job Details Our Community Hospice LPN’s: Work under the supervision and assignment of the registered nurse to provide top-notch care to hospice patients and their families within a home setting.  Communicate status of patient to team  Help patient manage pain and end of life symptoms  Experience in home care, skilled nursing facilities and/or oncology preferred. Minimum Qualifications: Minimum of one to two years prior bedside nursing experience required Current and valid NY State license to practice as a Licensed Practical Nurse Must have a current, valid New York State driver's license and own transportation Experience in home care, skilled nursing facilities Posted Salary Range USD $33.00 - USD $40.00 /Yr. This range serves as a good faith estimate and actual pay will encompass a number of factors, including a candidate’s qualifications, skills, competencies and experience. The salary range or rate listed does not include any bonuses/incentive, differential pay or other forms of compensation that may be applicable to this job, and it does not include the value of benefits.
