Palliative Care Referral RN

Summary Job Description: Palliative Care Referral RN Full-time (40 hours weekly), first shift Required to work every fourth weekend Job Description: The Palliative Care Referral RN will work with the Palliative Care team to support clinical staff and physicians in the management of the continuum of care for palliative care patients. This includes assisting with patient evaluation, level of care determination, patient management, and goals of care conversations. The RN will assist with the education of staff and physicians. Including assisting with completing advance directives and EMS DNR forms. Requirements: Associate's degree in nursing required. Active RN license through the Kentucky Board of Nursing Two years of acute care experience required. Must obtain CHPN within 3 years of hire Preferred Qualifications: Previous experience in palliative or hospice care preferred. Bachelor of Science in Nursing preferred.
