Multiple functions relating to direct patient care, patient education and advocacy, and care coordination. Facilitate program development, quality improvement, and community outreach. Key objectives of the position include: 1. Identify and eliminate barriers to care. 2. Support timely diagnosis/staging and treatment planning. 3. Provide educational support. 4. Anticipate patient needs and coordinated services to address current and future needs. 5. Coordinate appointments with a variety of health care providers. 6. Establish processes and standards that ensure consistent and optimal patient navigation. 7. Plan, develop and implement patient-centered programs. 8. Participate in continuous program development and quality improvement. Job Requirements, Prerequisites and Essential Functions: Job Specifications: Education: Graduate of an accredited baccalaureate school in a healthcare related field Nursing and oncology experience preferred Experience: With clinical credentials: 3-5 years of progressively more responsible clinical experience Oncology navigation experience preferred Without clinical credentials: 7-9 years of progressively more responsible healthcare experience Minimum of 3 years oncology navigation experience Prior experience leading groups of clinical and non-clinical staff License and Certifications: Basic Life Support Required at hire Integrative Agitation Management (IAM) Required within 30 days of hire Registered Nurse Preferred at hire Prerequisite Skills: Embodies all of Marin General Hospital’s C.A.R.E.S. standards. Intermediate to advanced computer skills. Must have working knowledge of Microsoft Office applications and electronic medical record systems. Exceptional inter-personal skills required for connecting with patients as well as successfully interacting with colleagues, internal physicians and staff, referring physicians and community organizations. Ability to work independently on prioritizing job responsibilities and multiple tasks around deadlines, troubleshooting and resolving issues, and following procedures without direct supervision. Highly attentive to detail and follow through. Proven project management skills. Multiple examples of successfully conceptualizing, designing and executing patient/client programs, including qualitative and quantitative post-evaluations Ability to provide own transportation for offsite outreach programs and events. Excitement for working in entrepreneurial, fast-paced environment. Must be available to work occasional nights and weekend. Bi-lingual (English & Spanish) is beneficial. Duties And Responsibilities: Essential (Not Modifiable) Navigate: Navigate recently diagnosed cancer patients to appropriate health care services in the Marin General Hospital (MGH) system and through the ins-and-outs of their illness, recovery, and survivorship. Coordinate with other health care providers to ensure seamless cancer care (e.g., pathologists, radiologists, oncology nurses, radiation therapists, dietitians, social workers, integrative practitioners, financial counselors, and administrative staff) Help patients communicate successfully with their physicians and
