We are currently seeking a motivated and experienced full-time Physical Therapist to join our team at a partnered Assisted Living Facility on the Upper East Side (NYC) . All levels of experience are encouraged to apply and will train the right new graduate.
Job Type: Full-Time w/ benefits
Pay: 90k-105k
Location: NYC - Upper East Side, 80th st. (Assisted Living Facility)

Responsibilities:
Provide 1:1 physical therapy treatment to residents within the facility
Evaluate patients and develop individualized treatment plans based on physician orders and patient needs
Deliver care in accordance with professional standards and the established plan of care
Lead a weekly group exercise class focused on mobility and functional wellness
Collaborate with facility staff and caregivers to support patient outcomes
Plan and implement therapy programs to improve or restore functional abilities, reduce limitations, and enhance quality of life
Perform and document initial evaluations, progress notes, and discharge summaries in a timely and accurate manner
Monitor patient progress and modify treatment plans as needed to achieve optimal results
Provide care with a strong understanding of geriatric and memory care populations
Physical Therapist Program Graduate
Licensed Physical Therapist in the state of New York or eligible.
The anticipated base salary range for this position is $90,000.00-$105,000.00. Salary is based on various factors, including relevant experience, knowledge, skills, other job-related qualifications, and geography. Medical, dental, vision, 401(k), paid time off, and other benefits are also available, subject to the terms of the Company's plan.
