Job Description: Community-Based Nurse (RN or LPN) – ACT Team   📍   Indianapolis, IN   🏥   Adult Community Services (ACT Program)   🕒   Full-Time | Monday–Friday   Job Summary   Looking for a nursing role where you can make a real impact—beyond the traditional clinical setting?   Join Adult & Child Health as a   Community-Based Nurse (RN or LPN)   on our   Assertive Community Treatment (ACT) Team , where   you’ll   provide care directly in the community to individuals who need it most.   In this role,   you’ll   support adults with   serious mental illness and co-occurring conditions , helping them   maintain   stability, improve health outcomes, and live more independently.
