What You'll Do As a Home Health RN, you'll provide compassionate, individualized care while managing your own patient caseload.
Responsibilities include:
Complete comprehensive patient assessments and develop individualized plans of care.
Manage your own caseload while coordinating care with physicians, therapists, social workers, and other interdisciplinary team members.
Perform skilled nursing visits, patient education, medication management, wound care, and disease management.
Evaluate patient progress and adjust care plans as clinical needs change.
Identify physical, emotional, social, and environmental factors impacting patient health.
Communicate significant clinical changes to providers and the care team.
Maintain accurate, timely documentation in accordance with regulatory and organizational standards.
Advocate for patients and families while promoting independence and quality of life.
Ensure compliance with HIPAA, Medicare Conditions of Participation, and agency policies.
What You'll Need Current, unrestricted Registered Nurse (RN) license
Minimum of one (1) year of nursing experience in an acute care, home health, hospice, skilled nursing, or comparable clinical setting
Current driver's license, reliable transportation, and auto insurance meeting state requirements
Willingness to travel throughout the assigned territory
Associate Degree in Nursing (ADN) required; BSN preferred
Home Health or Hospice experience preferred, but not required
HomeCare HomeBase (HCHB) experience preferred
Ability to frequently lift, push, pull, and support up to 50 pounds
