Description of Work The North Carolina Rural Health Transformation Program (RHTP) is a statewide initiative led by the Department of Health and Human Services (DHHS) Rural Health Transformation Program Office and guided by a cross-agency steering committee including the Office of Rural Health, Public Health, Health Benefits–Medicaid and Mental Health divisions. RHTP represents a projected $1B transformative investment to improve health outcomes and access to care for nearly 3 million rural North Carolinians that will deliver “ROOTS Hubs” to connect medical, behavioral and social services programs tailored to meet the needs of the populations in each of North Carolina’s six Medicaid regions; improve prevention and screening, chronic disease management, maternal health and nutrition; expand behavioral health and substance use services; modernize and sustain rural health workforce; advance value-based payments; and accelerate technological innovation, access and interoperability. The Position is responsible for providing outreach and navigator support for the statewide projected $1B federally funded Rural Health Transformation Program (RHTP). The Rural Health Transformation Outreach Coordinator serves as a convener and navigator for Initiative 6 of the Rural Health Transformation (RHT) grant program. This position builds and maintains strong relationships with rural medical providers, local partners, residents, community stakeholders, the RHTP Technical Assistance Center, Cooperative Extension offices, NC 211, digital literacy hubs, and both grantees and subgrantees. The Lead ensures cohesive messaging, coordinated technical assistance, and unified service delivery across all components of the program. Knowledge Skills and Abilities/Management Preferences Salary Grade Range: $67,122.00 - $117,465.00   Recruitment Range: $67,122.00 - $91,241.00  Candidates now meet the minimum qualifications of a position if they meet the minimum education and experience listed on the vacancy announcement. The Knowledge, Skills, and Abilities (KSAs)/ Management Preferences are not required. Applicants who possess the following skills are preferred: Demonstrated knowledge of digital technologies associated with healthcare delivery. Proven ability to convene diverse stakeholders and sustain collaborative partnerships. Strong verbal and written communication skills, with comfort engaging both clinical and non‑clinical audiences. Demonstrated project management and organizational abilities. Experience with rural health challenges and resources in North Carolina, with the ability to travel statewide—particularly to rural areas—for convenings, site visits, meetings, and community engagement activities. About the Rural Health Transformation Program NC Rural Health Transformation Program is a statewide program led by NCDHHS to transform rural health care delivery, improve health outcomes, and ensure sustainable access to care for nearly 3 million rural North Carolinians. The program is funded over five years by the Centers for Medicare & Medicaid Services (CMS). The main goals of the program are to:  Catalyze innovative health care models for rural communities by changing the way providers work together to care for patients in rural NC.  Transform the rural care experience, building community-based clinical, behavioral, and social supports close to home.  Create a sustainable rural health delivery system through underlying systems changes in rural workforce pipelines and care team models, and rural provider financial models.
