Role Scope :
This is an opportunity for a mission-driven team player who is passionate about attending to the daily living and safety needs of an assigned group of participants in our PACE center and occasionally in the home. By performing various participant care activities and related non-professional services, you will ensure our participants’ physical, social, and recreational needs are met so that they have the best experience possible when they are in our Habitat Health Center.

Meets participant’s personal hygiene such as bathing and other basic needs in a timely and caring manner.
Assists participants with activities of daily living, such as feeding, ambulation, toileting, and other activities.
Responds to and fulfills participant request or obtains appropriate assistance.
Performs and assists with assigned tasks in support of participant care.
Prepare and plate food for meal service, following instructions on participants’ dietary cards for specific diets, preferences, and special needs.
Ensures that the participant is comfortable, has necessary supplies within reach, and physical privacy is maintained.
Follows established standard operating procedures. Knowledge and performance of procedures would be acquired through continuing appropriate nursing education programs or supervised clinical experience.
Collects data and records activity and participant care information on appropriate forms. Ensures that information is accurate, timely and recorded according to established guidelines.
Provides participant instruction related to safety factors conducive to their care plan and recovery; demonstrates appropriate health, safety, and aseptic practices.
Cleans and straightens immediate participant surroundings ensuring that the participant is comfortable, necessary supplies are available and within reach, and physical privacy is maintained.
Assists in maintaining an orderly, clean participant care unit by straightening supply and linen storage, nursing station and equipment area.
On occasion, provides homemaking services in the home including preparing and serving meals and snacks according to specified nutritional needs and dietary restrictions as outlined on the plan of care. Performs light housekeeping tasks as outlined on plan of care including vacuuming, mopping, dusting, taking out trash, washing dishes and doing laundry.
Accompany participants for outside appointments as needed.
Observes participants for emotional, behavioral, and physical changes in condition such as any changes in mood, skin integrity, bowel and bladder status, food/fluid intake, speech, ambulation, and transfer ability. Promptly reports participant change of condition to nursing or group home staff.
Performs related duties as assigned.
Required Qualifications:
Completion of accredited vocational nursing assistant course, hospital, or comparable participant care experience. “Comparable experience” should include skills such as: 1) TPR; 2) BP; 3) bed bath; and 4) ADL.
High School diploma/GED
Knowledge of PACE program.
Ability to work without close supervision or professional guidance and to exercise independent judgment.
Knowledge of physical, mental, and social needs of frail older adults.
Able to manage changing priorities per participant needs.
Effective interpersonal skills to work with participants, caregivers, and staff.
Ability to work sensitively and effectively with individuals of diverse ethnic and cultural backgrounds.
Ability to be self-directed and able to work independently.
Requires physical strength to perform essential functions of the job (i.e. assist participants with mobility challenges).
A minimum of one year experience working with the frail or elderly.
Proof of medical clearance for communicable diseases, including a TB test.
Proof of all immunizations are up to date.
Proof of current CPR/BLS certification required or requirement to obtain within 30 days of employment.
Proof of valid CA driver’s license, personal transportation, good driving record and auto insurance as required by State law.
Preferred Qualifications:
Bilingual: Spanish/English preferred.
PACE experience and/or ADHC/CBAS experience
A state issued driver’s license, personal transportation, and auto insurance as required by law.
